Essential Person Centered Wrap Around Services: Understanding the total person is the first step in developing a viable behavioral solution, and is essential throughout the behavior management process. A full functional assessment is completed by a certified, person-centered practitioner (PCP), which includes research and background history, interviews and input from the person's circle of support, and observation and shadowing in natural environments (home, day setting, etc.) The information gleaned from assessment will be used in developing the behavioral management plan. The person-centered practitioner, as a member of the Essential Behavioral team, functions as a liaison and coordinator of key service elements within the behavior program. Throughout the process the PCP provides assessment, support, resource coordination, and advocacy and follow up to the consumer.
